No. You do not need a formal diagnosis to get started. Many families come to Memory Bridge while they are still waiting for answers. Dr. Carly can help you understand what you are observing, connect you with the right resources, and prepare you for your neuropsychological evaluation so you know exactly what questions to ask and feel confident getting the answers you need to move forward. You do not have to wait to get support.
Memory Bridge is a private-pay practice and does not bill insurance directly. All fees are due at the time of service. Upon request, Dr. Carly can provide a receipt that you may submit to your insurance carrier for potential out-of-network reimbursement.
Yes, and we encourage it. Sessions are designed to support both the caregiver and the person living with dementia. Dr. Carly welcomes loved ones to attend so that everyone leaves feeling informed, supported, and more confident about the path forward.
The Initial Consultation is your starting point. It focuses on understanding the diagnosis and building a foundation for future planning. Caregiver and Family Support Sessions are available to existing clients and are designed for ongoing questions as dementia progresses, including exploring dementia-focused approaches such as the Best Friends Approach.
Yes. Dr. Carly offers both telehealth and in-person appointments. Telehealth sessions are conducted through a secure, HIPAA-compliant video platform and are available to clients located anywhere in Minnesota.
Memory Bridge currently serves clients located in Minnesota, both in-person and via telehealth.
The Initial Consultation is the right starting point for new families. During that 90-minute session, Dr. Carly will learn about your situation in depth and help guide you toward the services that best fit your needs and goals. If you have questions before booking, you are welcome to reach out through the contact form.
Come prepared with a written list of your primary concerns, the diagnosis if one has been made, all questions you have related to future planning, and notes on any steps your family has already taken to prepare. The more you bring, the more Dr. Carly can help.
Primary care physicians are essential partners in dementia care, but appointments are often brief and focused on medical management. Dr. Carly specializes exclusively in dementia education, consultation, and support. She offers dedicated time to help you understand the diagnosis, navigate what comes next, and build a plan that works for your family.
Yes. Memory Bridge accepts HSA and FSA cards as payment. A receipt will be provided for your records after each appointment.
We understand that life with dementia is unpredictable. If you need to cancel or reschedule, please notify us at least 48 hours before your appointment. Cancellations made within 48 hours of the scheduled appointment, or missed appointments, will be charged 50% of the session fee. Please reach out as soon as possible and we will do our best to find a time that works for you.

We believe cost should never be a barrier to care. Dr. Carly offers a limited number of reduced-fee appointments for those in need. Please reach out to learn more.
